How to get services:

Contact your local Aging and Disability Resource Center

  • The first step is to contact the Aging and Disability Resource Center (ADRC) that serves your county or a tribal aging and disability resource specialist (ADRS). 

  • How they help: The ADRC will explain your options for long-term care, determine your eligibility, and walk you through the enrollment process.

  • Find your local ADRC: You can find the contact information for your local ADRC on the Wisconsin Department of Health Services website or by calling 844-WIS-ADRC (844-947-2372).

Undergo eligibility screening through ADRC

  • With assistance from the ADRC, you will complete two types of eligibility screenings for the Family Care program: 

    • Functional eligibility: A specialist will conduct a Long-Term Care Functional Screen to determine your level of need for services.

    • Financial eligibility: You will also need to meet certain income and asset limits, which the ADRC can help you apply for through an income maintenance agency.

Choose a Managed Care Organization (MCO)

  • If you are found eligible for the Family Care program, you will be offered a choice of MCOs to receive your benefits from. 

  • After selecting an MCO the ADRC will then finalize your enrollment and notify the MCO.

    • Clearwater works exclusively with Inclusa, and MyChoice WI. ​

Begin services with MCO

  • After your enrollment is complete, the MCO will reach out to you to begin the process of planning your services. 

  • Develop a plan: You will work with a dedicated MCO care coach, and RN, to create a personalized plan of services.

  • At this point you can request Clearwater as your CSL provider, if deemed eligible.
